I understand the physical aspects of changing the aperture and shutter speed parts of the exposure triangle, but with ISO gain, does the sensor sensitivity just change within the camera? Also for clarification, with "better" cameras, you can increase the ISO gain without causing as much noise, is this due to a sensor that can handle more sensitivity?

For "better" cameras, the sensors can have more pixels which can capture more light and provide better image quality at higher ISOs.
