This image makes it seem that the t-coordinate increases upward, as opposed to the conventional graphics representation with the y-coordinate starting at the top and increasing downward. Are textures stored differently from images or is this image just for a demonstration of the mapping?

I think they just used indices for the points that happened to increase that way, the indices don't necessarily correspond to the actual coordinates of the points.
